Motor Tax Forms

General Information for Applicants

Please click on the link for the form you require. In some cases you will be redirected to the Road Safety Authority Website to access the relevant form.

The application form must be completed in full, using a black ballpoint pen. Each section must be completed. Your particular attention is drawn to part 6 of the application form for a full licence D401 or part 5 of the application form for a learner permit D201, titled “Declaration by Applicant”. It is essential that you complete and submit this part of the form.

If all the required documentation and appropriate fee is not received, your application will be returned to you which will lead to an unnecessary delay in processing your application.

Please refer to the detailed guidance notes for completion of Form D201 and D401 which are included on each form. Please also refer to the notes set out in the next page which may assist you in making your application.

Photographs to accompany application for driving licence/learner permit.

Please ensure that the photographs you submit are fully in accordance with the requirements set out in paragraph 8 of form D201 or D401.The Road Safety Authority has not designated specific locations for the taking of driving licence/learner permit photos
